It turns out it was a game variable initialization script that was treating saved game data as though it were unique per saved file. Glad to put this one behind me and allow you keep the settings you prefer. The following changes have been made to Transliminal: Beyond The Backrooms v0.0.98648a:

removedTweaks & FixesDefeating a Smiler no longer grants +1 Maximum Sanity every time. The first defeat still grants +1, but each subsequent Maximum Sanity increase from Smilers requires one additional defeat than the last.
fixedTweaks & FixesFixed issue with random instantiation script that would not instantiate prefabs if the list did not loop. This was stopping several game elements from appearing.

Tweaks & Fixes

  • Finally fixed the issue where data initialization script was causing custom settings to be overwritten to defaults when creating a new saved game or loading a different saved game. Now you can save your settings with confidence!

  • Defeating a Smiler no longer grants +1 Maximum Sanity every time. The first defeat still grants +1, but each subsequent Maximum Sanity increase from Smilers requires one additional defeat than the last.

  • Fixed issue with random instantiation script that would not instantiate prefabs if the list did not loop. This was stopping several game elements from appearing.

  • Changed Mother Doll mechanics. 64 dolls must be defeated and the assembled doll must also be burned for Mother’s Fingers to be vulnerable in the Castle.

  • Baby Doll counter now tracks how many Dolls have been defeated and only appears when a Doll is defeated.

  • Dolls can no longer deal damage after they have been phased

  • Increased the number of times Victor warns about Energy before he passes out and warps to another area.

  • When the PlayerController is Disabled it now stops all motion and velocity values.

  • Crouching and standing is no longer instant and now has a fast but smooth transition.

  • Fixed script value that was preventing Ceiling Smilers from spawning

  • Fixed issue that was preventing Victor from commenting on the level

  • Better and more frequent placement of clipped objects in Level 0 walls

  • More even volume on step sound effects

Okay. Lot's of new & exciting stuff coming down the pipe that I think you're really going to enjoy.
