This is Ota, the director. Since release, we have received several reports from players saying they cannot collect all the "Memory Fragments" or are unsure where they may have missed one. We sincerely apologize for the inconvenience.
On the development side, we are currently considering an additional patch that would make uncollected "Memory Fragments" easier to identify for players. We will share more details as soon as the patch is ready.
As an immediate workaround, we have prepared a spreadsheet that summarizes the acquisition conditions for every "Memory Fragment." If you are having difficulty completing the collection, please feel free to refer to it. ※ Please note that this resource contains significant spoilers related to the main story. Please view it at your own discretion.
