Removed NG+ inheritance of unique accessories

Developer's comment: the girls' unique accessories kept duplicating (Vera's Lucky Coin, for example), so we cleaned up inheritance for those and for the Amulets.

Other fixes

Fixed a crash in the NG+ questlog caused by Noblesse Oblige
Fixed a crash in the NG+ questlog caused by Fake it till you make it
Fixed the rewards and descriptions of some quests auto-completed in NG+
Fixed a bug that would remove an inherited Armlet from NG+

Unfortunately, most of these fixes are not retroactive —they will be applied to new savefiles, but not existing ones.
