In Options->Audio, Moans, press the button to see the list of all moan themes. Those with the magenta download button are paid. Before purchase, press play (triangle) button a few times to hear different random audio files from the theme, just to make sure you like those sounds.
DLCs now use Steam overlay and also should normally download DLC content after purchases without the need to restart the game.
